Program pathways

The following flowchart sets out the process by which participants can be referred into the Life! Program.

For patients 40 years and above and all adult Aboriginal and Torres Straight Island people

People of any age may be referred to a Life! course through the WorkHealth initiative if they are at high risk of diabetes.
